LPN MDS Coordinator

Genesis HealthCare, a leading provider of healthcare services, is seeking an LPN MDS Coordinator to join our team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing items on the Minimum Data Set (MDS) to ensure appropriate patient care and nursing center reimbursement.

Position Highlights

• Direct the RAI process and ensure compliance with guidelines for MDS completion and electronic transmission.
• Develop individualized resident Care Plans in line with Federal and State regulations.
• Train interdisciplinary team members on clinical content and MDS completion.

Requirements

• LPN in good standing with current state licensure.
• Prior experience in healthcare settings.
• Proficient in general, rehabilitative, and restorative nursing practices.
• Computer skills with knowledge of Windows applications.
• AccuMed experience preferred.
• Previous experience as an MDS Coordinator is a plus.
