Job Description

We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker at Levindale Hospital in Baltimore, MD, you will be responsible for providing clinical social work services and psychosocial assessments to patients/residents and families. Your role will involve addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services.
• Address common problems such as adjustment issues and family relationships.
• Assist patients/residents and families with financial concerns and discharge planning.
• Provide counseling related to death and dying.

Requirements

• Bachelor's in Social Work (BSW) required.
• Master's degree in Social Work preferred.
• LCSW-C certification preferred.
• LGSW certification required.
