Job Description
ChristianaCare HomeHealth is looking for skilled Licensed Practical Nurses (LPNs) to join our team in New Castle County, DE. As part of our team, you will provide geriatric patients with skilled nursing care, implement personalized care plans, and contribute to discharge planning. We offer Full Time, Part Time, and Per Diem positions with day or night shifts, one weekend a month, and 2 holidays a year.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must hold a current DE LPN license or compact state license.
