Responsibilities

• Provide skilled nursing care and treatment to geriatric patients in the community.
• Implement patient-centered care plans and assist in discharge planning.
• Support New Castle County, DE, with day/night shift work options, one weekend a month, and 2 holidays per year.

Requirements

• High School Diploma (HSD) or GED required.
• Current DE LPN license or compact state license.
