Job Description
We are looking for a dedicated Home Care LPN to join our ChristianaCare team. As part of our HomeHealth division, you will provide skilled nursing care to geriatric patients in the community. Your role will involve implementing care plans, participating in discharge planning, and ensuring patient-centered care.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented care plans consistent with available resources.
- Contribute to developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
