Job Description
ChristianaCare HomeHealth is looking for an experienced Home Care LPN to join our team in New Castle County, DE. As an integral part of our organization, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning.
Responsibilities
- Give skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans.
- Participate in developing patient discharge plans.
Requirements
- HSD or GED required.
- Current DE LPN license or compact state license.
