Job Description
We are currently looking for an experienced Home Care LPN to join ChristianaCare's HomeHealth team. If you are a dedicated Licensed Practical Nurse with a passion for providing skilled nursing care to geriatric patients in the community, we invite you to apply. As part of our team, you will have the opportunity to work in New Castle County, DE, on a full-time, part-time, or per diem basis, contributing to goal-oriented care plans and ensuring patient-centered treatment.
Responsibilities
- Provide skilled nursing care and prescribed treatments to geriatric patients.
- Implement and contribute to goal-oriented care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
