Job Description
We are looking for an experienced Home Care LPN to join ChristianaCare's HomeHealth team. As an integral part of our organization, you will provide skilled nursing care to geriatric patients in the community, participate in care planning, and support patients on their road to recovery. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in coordination with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
