Job Description
We are looking for dedicated Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. As an LPN, you will have the opportunity to provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning, all while following patient-centered and goal-oriented approaches.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement care plans that are patient-centered and consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must possess a current DE LPN license or compact state license.
