Licensed Practical Nurse MDS Coordinator

Job Description

Genesis Health is seeking a motivated Licensed Practical Nurse MDS Coordinator to join our team. As an integral part of the Resident Assessment Process, you will be responsible for completing relevant items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Direct and oversee the interdisciplinary RAI process, ensuring compliance with guidelines for MDS completion and transmission.
• Participate in developing individualized Care Plans based on resident needs and regulatory requirements.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in a healthcare facility, preferably in long-term care.
• Familiarity with nursing practices, medical procedures, and long-term care regulations.
• Proficiency in Windows applications; AccuMed experience is a plus.
• Previous MDS Coordinator experience preferred.
• Current BLS/CPR certification.
