We are seeking dedicated and experienced Licensed Practical Nurses (LPN) to join our Home Care team at ChristianaCare. As an LPN in our HomeHealth division, you will have the opportunity to provide skilled nursing care and treatment to geriatric patients in the community. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans in alignment with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
