Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our HomeHealth team at ChristianaCare. As an LPN in our HomeHealth division, you will provide skilled nursing care and treatment to geriatric patients in the community. The successful candidates will implement goal-oriented care plans, participate in discharge planning, and work flexible hours to support our patients' needs.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in the development of discharge plans.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
