Genesis Health is looking for a motivated Minimal Data Set Coordinator who is a Licensed Practical Nurse to join our team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans following Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items.

Requirements

• Active LPN license.
• Prior experience in a healthcare environment.
• Proficiency in general, rehabilitative, and restorative nursing practices.
• Computer literacy with Windows applications.
• AccuMed experience preferred.
