Job Description

We are looking for a dedicated Minimal Data Set Coordinator - LPN to join Genesis Health, a leading healthcare services provider committed to delivering high-quality care and compassion to improve lives. As the LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure proper patient care and nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process in compliance with guidelines for MDS completion, computer encoding, and electronic State transmission.
• Contribute to the development of personalized resident Care Plans, identifying resident needs and expected outcomes as mandated by Federal and State regulations.
• Provide training to new team members on clinical content and electronic completion of relevant MDS items.

Requirements

• Active LPN license with relevant experience in healthcare settings.
• Familiarity with general, rehabilitative, and restorative nursing practices, along with knowledge of medical procedures and regulations in long-term care.
• Proficiency in windows applications; experience with AccuMed is a plus.
• Prior experience as an MDS Coordinator and demonstrated expertise in RAI/PPS processes.
• Current BLS/CPR certification required.
