Genesis Healthcare is looking for a dedicated Nurse-LPN MDS Coordinator to join our team. As an LPN MDS Coordinator, you will play a vital role in the Resident Assessment Process by completing necessary items on the Minimum Data Set (MDS). Your expertise will ensure appropriate patient care and nursing center reimbursement, making a significant impact on our residents' well-being.

Position Highlights

• Lead the RAI process and ensure compliance with MDS guidelines.
• Develop individualized Care Plans in accordance with Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• Active LPN license.
• Prior experience in healthcare settings.
• Proficiency in general nursing practices and MDS completion.
• Computer literacy, including Windows applications.
• AccuMed experience is a plus.
