Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit assessments to assess medical conditions, limitations, and potential for rehabilitation.
• Proficient in EMR documentation.
• Hold CPR Certification.
