Paradigm Rehab is looking for a dedicated Physical Therapist to join our team and make a difference in the lives of our community members. As a Physical Therapist, you will work closely with patients to assess their medical conditions, functional capabilities, and potential for rehabilitation through home visit assessments.

Responsibilities

• Conduct home visit Physical Therapy assessments to provide direct patient care.
• Document patient information accurately in EMR systems.
• Collaborate with patients to develop personalized rehabilitation plans.

Requirements

• Graduate from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license for New York.
• Proficient in EMR documentation.
• CPR Certification.
