Responsibilities

• Conduct home visit assessments of referred patients.
• Provide direct patient care to assess medical conditions and functional capabilities.
• Document patient information using EMR.
• Maintain a compassionate and professional approach in patient interactions.

Requirements

• Graduated from an accredited Physical Therapy program.
• Hold a valid Physical Therapy license for New York.
• Proficient in EMR documentation (experience with Hello Note is a plus).
• CPR Certification.
