Physical Therapist Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Perform home visit assessments, evaluate medical conditions, and provide direct patient care.
• Proficient in EMR documentation.
• Current CPR Certification.
