Physical Therapy Specialist Responsibilities:

• Conduct home visit assessments and provide direct patient care.
• Assess medical conditions, functional capabilities, limitations, and rehabilitation potential.
• Proficient in EMR documentation.
• Maintain an unencumbered Physical Therapy license for New York.
• Hold CPR Certification.

Requirements

• Graduated from an accredited PT college or university.
• Experienced in conducting Physical Therapy assessments.
• Comfortable working with Hello Note EMR system.
