Responsibilities

• Conduct comprehensive Physical Therapy assessments during home visits.
• Provide direct patient care to evaluate medical conditions, functional capabilities, and rehabilitation potential.
• Utilize EMR documentation efficiently (experience with Hello Note is a plus).
• Maintain CPR Certification and uphold professional licensing standards.

Requirements

• Graduation from an accredited Physical Therapy (PT) program.
• Valid, unencumbered Physical Therapy (PT) license for New York.
• Ability to relocate to Brooklyn for in-person work.
• Strong interpersonal and communication skills.
