Requirements

• Graduate of an accredited Physical Therapy (PT) program.
• Hold a valid Physical Therapy license for New York.
• Conduct in-home assessments, establish care plans, and document patient progress.
• Proficiency in EMR documentation is preferred.
• CPR Certification required.

Salary
