Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct in-home Physical Therapy assessments, providing direct patient care and rehabilitation potential evaluations.
• Proficiency in EMR documentation; experience with Hello Note is a plus.
• CPR Certification.
