Responsibilities

• Evaluate and assess patients' medical conditions, functional capabilities, and potential for rehabilitation.
• Deliver personalized physical therapy sessions to support patients in their recovery journey.
• Maintain accurate and timely documentation using our EMR system.

Requirements

• Graduation from an accredited Physical Therapy (PT) program.
• Valid, unencumbered Physical Therapy (PT) license for New York.
• CPR Certification.
• Ability to conduct thorough home visit assessments and provide direct patient care.
