Responsibilities

• Conduct home visit assessments and provide direct patient care.
• Evaluate medical conditions, functional capabilities, and potential for rehabilitation.
• Proficiently document patient information utilizing EMR systems.

Requirements

• Graduation from an accredited Physical Therapy (PT) program.
• Valid, unencumbered Physical Therapy (PT) license in New York.
• CPR Certification.
• Experience with EMR systems preferred.
