As a Physical Therapist, you will play a vital role in helping individuals reconnect with their environment and enhance their quality of life. Responsibilities

• Conduct home visit Physical Therapy assessments and provide direct patient care.
• Evaluate medical conditions, functional capabilities, limitations, and rehabilitation potential.
• Proficient in EMR documentation (experience with Hello Note is a plus).

Requirements

• Graduate from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Current CPR Certification.
