Responsibilities:

• Conduct home visit assessments to evaluate patients' conditions and rehabilitation potential.
• Provide direct patient care and develop treatment plans.
• Document patient progress using EMR systems.

Requirements:

• Graduation from an accredited PT program.
• Valid Physical Therapy license for New York.
• Proficiency in EMR documentation.
• CPR Certification.
