Rehabilitation Therapist Requirements:

• Graduation from an accredited Physical Therapy (PT) college or university.
• Unencumbered Physical Therapy (PT) license for New York.
• Perform home visit Physical Therapy assessments, providing direct patient care and evaluating medical conditions, functional capabilities, limitations, and rehabilitation potential.
• Proficiency in electronic medical records (EMR), experience with Hello Note is a plus.
• CPR Certification required.
