SOCIAL WORKER

Job Description

We are looking for a passionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing various challenges they may be facing. This role involves assisting individuals and families with adjustment to institutional living, financial concerns, family relationships, medical treatment comprehension, aging-related issues, discharge planning, and end-of-life counseling.

Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments to identify needs and challenges.
• Offer counseling and support for emotional difficulties and decision-making.
• Collaborate with healthcare professionals to ensure comprehensive care.

Requirements

• Bachelor's degree in Social Work (BSW) required.
• Master's degree in Social Work preferred.
• Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
• Licensed Graduate Social Worker (LGSW) certification.
