Job Description
We are looking for a dedicated and compassionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families. This role involves addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to difficult life situations.
Responsibilities
- Conduct psychosocial assessments of patients and families.
- Provide counseling and support to individuals facing medical diagnoses and treatment.
- Assist with discharge planning and navigate end-of-life care.
- Collaborate with healthcare teams to ensure holistic patient care.
Requirements
- Bachelor's degree in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- Licensed Certified Social Worker (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
