Job Description
We are looking for a dedicated Social Worker to join LifeBridge Health in Baltimore, MD at Levindale Hospital. As a Social Worker, you will be responsible for providing quality clinical social work services and conducting psychosocial assessments for patients and their families. This role involves addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, discharge planning, and counseling on end-of-life issues.
Responsibilities
- Conduct psychosocial assessments for patients and families.
- Provide counseling on various social and emotional challenges.
- Assist with discharge planning and support patients through transitions.
- Collaborate with healthcare teams to ensure holistic care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) credential.
