Job Description
We are looking for a compassionate and dedicated Social Worker to join LifeBridge Health in Baltimore, MD. As part of our allied health team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common issues from adjustment to institutional living to counseling related to the end of life.
Responsibilities
- Conduct psychosocial assessments for patients and families.
- Provide counseling and support for emotional and practical challenges.
- Develop discharge plans and coordinate post-hospital care.
- Collaborate with healthcare professionals to ensure holistic patient care.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification.
- Prior experience in clinical social work.
