Job Description
We are looking for a dedicated Social Worker to join LifeBridge Health's team at Levindale Hospital in Baltimore, MD. In this full-time role with day shift hours, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may face. Responsibilities
- Provide counseling and support to patients and families on issues such as adjustment to institutional living, financial concerns, and discharge planning.
- Assess and address emotional and psychological needs related to medical diagnoses, aging, and end-of-life care.
- Collaborate with healthcare teams to develop comprehensive care plans.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree in Social Work preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
