Job Description

We are looking for a compassionate Social Worker to join our team at LifeBridge Health. In this full-time position based in Baltimore, MD, you will provide essential clinical social work services and psychosocial assessments to patients/residents and their families. Your role will focus on addressing common challenges such as adjustment issues, financial concerns, family dynamics, and end-of-life counseling. If you are a Bachelor's in Social Work (BSW) with a preference for a Master's degree, and hold certifications such as LCSW-C or LGSW, we encourage you to apply.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address issues related to adjustment, financial concerns, family relationships, and end-of-life counseling.
• Collaborate with patients, families, and healthcare teams.
• Participate in discharge planning and counseling.

Requirements

• Bachelor's in Social Work (BSW) required; Master's degree preferred.
• LCSW-C or LGSW certification preferred.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a healthcare setting.
